Everyone is welcome. You will be asked to fill out a brief health questionnaire at your first appointment. Please list any medications you are taking. Some medications and massage do not mix. Serious health conditions may require a physician approval before your massage. Feel free to call and discuss this with me, should you have any questions. Examples would be, but not limited to, circulatory problems, injury, high-risk pregnancy, uncontrolled low or high blood pressure. Please be as thorough as possible. It's in your best interest. On subsequent appointments, a brief update is all that is asked.
Appointment and cancellation policy:
All massages are given by appointment only, appointments times are varied. I strive to be available on the days and times that are best for you, including days, evenings, Saturday by appointment, some holidays.
Your appointment time, once scheduled, is being held for you. I am glad to reschedule your appointment should the need arise. I simply request that if you cannot be at your scheduled appointment that you let me know by phone as soon as possible, 360-319-3924.
Please leave a message if I do not pick up, as I may be in session with another client. Any no-shows or appointments canceled within twenty four hours of the scheduled appointment time will be billed in full for the missed session. Insurance does not cover no-shows or missed appointment fees. If you are an insurance customer, you will be responsible for those charges.
Payment is due at the conclusion of each massage session, unless I have approved other arrangements prior to your appointment. I accept cash, checks, Visa, Master Card, Discover, American Express, and my gift certificates. Gift certificates have an expiration date of one year, though I encourage their use as soon as is practical. Returned check fee is $25.00. Accounts not paid within terms are subject to a 1.2% monthly finance charge.
I also accept L&I and PIP insurance, after you have provided all necessary insurance billing information, and a prescription from a qualified physician, chiropractor, etc. listing the codes indicating the muscles/areas to be treated by massage, the prescribed number of treatments, and dates the treatments are to begin and end.
I am not an approved provider for any HMO's, Blue Cross/Regent, or any other medical insurance groups. Though it may be nice to have your treatments covered by insurance, there are benefits to paying for your own. The best part of paying for your own treatment is that you and I can choose how you would like to spend your massage session, unrestricted by a prescription. When an insurance company pays for your massage, I legally cannot do a full-body massage, unless prescribed as such. My massage is legally restricted to the muscle groups indicated by your prescription

Termination of session policy:
Any inappropriate remarks, gestures, behaviors, or actions will not be tolerated, and will result in the immediate termination of the massage session. Payment in full for the appointment, whether completed or not, will be immediately due. You will be asked to leave the premises immediately. Refusal to pay will result in collection costs being added to your session. Obviously, you will not be welcome to return.
